Registering with the
Department of Health
Prior to having your certification appointment with a Doctor, you must register with the Pennsylvania Department of Health. This registration process requires a state issued drivers license or ID card. They confirm that you are a registered state resident and issue you a 6-digit patient ID number. This number is required for your New Patient appointment so that the Doctors can find your account with the Department of Health to certify you qualify for a medical card. How to Get Your
Medical Card

Register with PA Department of Health
Click on the button below to take you to the Pennsylvania Department of Health website. Here you will register with their system to confirm that you are a qualifying state resident. You must have a state issued ID or Drivers License to participate in the PA Medical Card program. They will assign you a 6-digit patient ID number. Please make sure that your most current address is listed on the Department of health registration prior to your appointment with our providers. Schedule Certification Appointment with a Doctor
Click on the button below to take you to our scheduling page. After you have register with the PA DOH you must be certified that you medically qualify for the program by a Doctor. You can select a telephone appointment time and make payment through our scheduling portal below. You will receive a phone call from one of our Doctors within 30 minutes of your selected appointment time.

Appointment and Finalization
After your scheduled appointment, you will receive an email from the DOH. You must open and follow the instructions on this email. If you owe the state any fees for your card this will be when they are processed and where any fee waivers are confirmed. The DOH will then print and mail your card. Please allow 2 weeks for it to arrive via USPS. Upon arrival you may visit any dispensary in the state of Pennsylvania to purchase product.
* If this is your first time purchasing with your medical card, we recommend that you make an appointment with a pharmacist at your local dispensary for assistance selecting the correct product for your needs.

Cancer, including remission therapy

Damage to the nervous tissue of the central nervous system (brain-spinal cord) with objective neurological indication of intractable spasticity, and other associated neuropathies

Dyskinesis and spastic movement disorders

Glaucoma

HIV / AIDS

Huntington's disease

Inflammatory bowel disease

Intractable seizures

Multiple sclerosis

Neurodegenerative diseases

Opioid use disorder for which conventional therapeutic interventions are contraindicated or ineffective, or for which adjunctive therapy is indicated in combination with primary therapeutic interventions

** Diagnosis change slightly between Maryland and Pennsylvania. Our provider certify for both states.
